INSURANCE
Evans Urgent Care participates in most major insurance plans. Since the list of insurance carriers is constantly growing, please contact our office by phone to verify that we are in-network with your insurance provider. As a courtesy to our patients, we file all insurance claims. Since Evans Urgent Care is not an emergency room all claims will be filed as regular primary care physician office visits.
Many insurance plans require their members to contribute to their health care costs with an out-of-pocket payment called a co-payment. All co-payments and coinsurance are collected at check-in at the time of service.

Surgery:
Remove 1-2 simple skin tumors without stitches (shaved off - for skin tags and moles)

Add $20 to every skin tumor without stitches > 2 to a max of 5 per day.
Remove 1-2 simple skin tumors with stitches (deep moles, warts and other lesions)

Credit Card/Check: $250 (may use a check if well established and Office Manager approves)
Add $35 to every skin tumor with stitches > 2 to a max of 3 due to being more time consuming.
BILLING & PAYMENT
You are responsible for paying any deductible, co-insurance or other balance not paid by your insurance. All payments are due at time of service. Please speak with our practice manager if you need to discuss payment issues.
We accept all major credit and debit cards, as well as cash and personal checks.
